Penile Enlargement
Honest assessment first. Many men who ask about enlargement have normal anatomy — and a treatable concern that surgery would not address.
Understanding Penile Enlargement
Concerns about penile size are common and genuinely distressing, but they are frequently based on inaccurate expectations rather than on abnormal anatomy. A substantial proportion of men seeking enlargement fall entirely within the normal range on objective measurement.
Our approach begins with proper assessment rather than a sales pitch. We measure objectively, identify whether a genuine anatomical issue exists, and screen for the conditions that often lie behind the request — erectile dysfunction, buried penis from obesity, Peyronie's shortening, or body dysmorphic concerns that surgery would only worsen.
Where a genuine indication exists, established procedures can help. Where it does not, we say so plainly. This is an area of medicine with a great deal of aggressive and misleading advertising, and men are routinely sold procedures, devices and injections that carry real risk of permanent harm.
Symptoms
- Persistent distress or preoccupation regarding penile size
- Avoidance of intimacy or relationships due to size concerns
- A penis that appears buried within the suprapubic fat pad
- Shortening following Peyronie's disease, surgery or prosthesis implantation
- Genuine micropenis, which is a rare and objectively defined condition
- Deformity or irregularity following filler injections performed elsewhere
If you recognise several of these, an assessment is worthwhile. Early evaluation usually means simpler treatment.
Causes & Risk Factors
Normal anatomical variation
The most common finding. Objective measurement usually confirms a size well within the normal range, and reassurance with accurate information is the appropriate treatment.
Buried or concealed penis
Excess suprapubic fat conceals a substantial portion of the shaft. Weight reduction often reveals two to three centimetres without any surgery at all.
Peyronie's disease
Plaque formation causes measurable shortening and curvature, and is treated as a disease rather than a cosmetic issue.
Post-surgical shortening
Following radical prostatectomy or prosthesis implantation.
Congenital micropenis
A hormonally defined condition confirmed by measurement against standardised norms, which requires endocrine evaluation.
Complications of unregulated procedures
Silicone, oil or unapproved filler injections cause nodules, infection, disfigurement and, in some cases, tissue loss requiring reconstructive surgery.
Body dysmorphic disorder
A psychological condition in which surgery reliably fails to resolve the distress and may deepen it. Recognising this is part of responsible assessment.
Diagnosis at RAMS
We diagnose before we treat. Each step below is chosen for what it will actually change about your management.
Objective measurement
Stretched flaccid length measured from the pubic bone, which correlates closely with erect length, compared against established population data.
Assessment for concealed penis
Evaluation of the suprapubic fat pad and skin attachments to determine how much length is present but hidden.
Erectile function assessment
Size concerns frequently accompany erectile dysfunction, and treating the ED often resolves the underlying dissatisfaction entirely.
Peyronie's evaluation
Examination for plaque, with Doppler assessment where curvature or shortening is present.
Hormonal evaluation
Testosterone and related hormones where genuine micropenis or delayed development is suspected.
Psychological screening
Assessment for body dysmorphic disorder, since operating on such patients produces poor outcomes and persistent dissatisfaction.
Treatment Options
Treatment is matched to the cause and to your circumstances. Every option, including doing nothing for now, is explained before you decide.
Accurate counselling and reassurance
For the majority of men, objective measurement and honest information resolve the concern without any intervention. This is a legitimate and frequently the best outcome.
Weight reduction
Where the penis is concealed by suprapubic fat, weight loss produces genuine visible length gain safely and without surgery.
Suprapubic lipectomy
Surgical removal of the fat pad in selected men with a significantly buried penis and stable weight.
Ligamentolysis (suspensory ligament release)
Releases the suspensory ligament to expose additional flaccid length. The gain is modest, applies mainly to the flaccid state, and the limitations are explained in full before any decision.
Girth enhancement
Autologous or approved graft techniques in carefully selected patients. Unregulated silicone, oil and industrial filler injections are never used and are strongly advised against.
Treatment of the underlying condition
Managing Peyronie's disease, erectile dysfunction or hormonal deficiency, which is frequently what the man actually needs.
Correction of previous complications
Removal of injected material and reconstructive repair following procedures performed elsewhere.
Benefits of Treatment
- An honest, objective answer rather than a sales pitch
- Detection of treatable conditions behind the concern
- Avoidance of unnecessary and irreversible surgery
- Safe, established techniques where intervention is genuinely indicated
- Correction of harm caused by unregulated procedures

Penile Enlargement FAQs
Do enlargement pills, pumps and devices work?
Pills have no proven effect on size. Vacuum devices produce only temporary change and can cause tissue injury with overuse. Traction devices have limited evidence in specific medical contexts, such as Peyronie's disease, rather than as general enlargement.
Are filler injections safe?
Silicone, oil and unapproved industrial fillers are dangerous and can cause permanent disfigurement, infection and tissue loss. We do not perform them, and we regularly see men needing corrective surgery after having had them elsewhere.
Will surgery give a dramatic increase?
No. Realistic gains from established procedures are modest, and largely affect the flaccid appearance rather than erect length. Any clinic promising dramatic results is not being honest with you.
What if my measurements turn out to be normal?
We will tell you so directly, and explain why. For many men this is a genuine relief. If the distress persists despite normal anatomy, that is worth addressing in its own right — surgery would not resolve it.
Can this be treated confidentially?
Yes. Consultations are private and records are access-controlled, as with every service we provide.
